IO Signature Request Form

A request form must be completed for any document (e.g., certificate of confidentiality for non-NIH funded studies, genomic data sharing institutional certification) that requires the signature of the TAMU VPR Institutional Official (IO) related to human subject research. The request form collects basic information and can be located here.

You will be contacted by the HRPP staff once your requested has been received. Please allow up to 72 hours for a response to your request.

NOTE: This request form may not be used to obtain institutional signatures for IRB Authorization Agreements or Individual Investigator Agreements
